I understand that a properly trained dog remains under the control of its handler. I understand that a properly trained dog does not act aggressively by biting, barking, jumping, lunging, or injuring people or other animals. It also does not urinate or defecate on the aircraft or in the gate area.

I understand that if he/she shows that it has not been properly trained to behave in public, then the airline may treat him/her as a pet by charging a pet fee and requiring him/her to be transported in an FAA-approved pet carrier.

To the best of my knowledge, he/she has not behaved aggressively or caused serious injury to another person/dog.

I understand that he/she must be harnessed, leashed, or tethered at all times in the airport and on the aircraft.

I understand that if he/she causes damage, then the airline may charge me for the cost to repair it, as long as the airline would also charge passengers without disabilities to repair the similar kinds of damage.

I am not registering for fraudulent misrepresentation of a Service Animal, and understand that my dog must meet the legal requirements in order for this registration and identification materials to be valid. I acknowledge that fraudulently misrepresenting an animal as a Service Dog may constitute a criminal offense in many states, and I assume full legal and financial responsibility for any misrepresentation. USSAR, LLC bears no liability for any fraudulent, illegal, or improper use of any registration, certificate, identification materials, or documentation provided.
I understand that both federal and local laws regarding Service Dogs are subject to change at any time without notice. USSAR, LLC is under no obligation to notify me of any such changes and accepts no liability for my reliance on any information that may become outdated. I will continue to abide by all current regulations applicable to Service Dogs.
USSAR, LLC makes no representations or warranties, express or implied, regarding the accuracy, completeness, fitness for a particular purpose, or legal sufficiency of any documentation, identification materials, or information provided. Should any access or privilege issues arise, I hold USSAR, LLC and its officers, directors, employees, agents, and assigns harmless, and understand USSAR, LLC can provide no guarantees that landlords, businesses, airlines, or any other third parties will be compliant and non-discriminatory with access and privileges protected by laws regarding Service Dogs. To the fullest extent permitted by applicable law, USSAR, LLC's total cumulative liability to me for any and all claims arising out of or related to this registration shall not exceed the total amount paid by me for this registration. In no event shall USSAR, LLC be liable for any indirect, incidental, special, consequential, punitive, or exemplary damages.
I agree to indemnify, defend, and hold harmless USSAR, LLC, its officers, directors, employees, agents, and assigns from and against any and all claims, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or related to: (a) my use of any registration or identification materials; (b) any misrepresentation made by me; (c) my violation of any applicable law; or (d) any third party's claims related to my Service Dog.
Any dispute arising out of or relating to this agreement shall be resolved by binding arbitration in the State of DELAWARE, under the rules of the American Arbitration Association. I waive any right to a jury trial and agree not to participate in any class action lawsuit or class-wide arbitration against USSAR, LLC. This agreement shall be governed by the laws of the State of DELAWARE, without regard to conflict of law principles. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
