Here's how the USAR application works, start to finish, and what you'll need to fill it out.

First — what you're actually applying for

The ADA does not require any service dog application or registration. You don't need permission from any government to have a service dog. What you can apply for is a documentation toolkit: a printed photo ID card, a Wallet pass, a public verify URL, and (optionally) related documents like a housing letter or DOT air-travel form.

What you'll need before you start

  • Handler info: your full name, mailing address, email, phone
  • Animal info: dog's name, breed, color, approximate weight, date of birth (or estimated age)
  • Photos: a clear photo of your dog (used on the printed ID card and in the Wallet pass) and a photo of yourself for the Handler ID
  • Registration type: Service Dog, Emotional Support Animal, or Psychiatric Service Dog
  • State of residence: for state-law tagging in your registration record
  • Payment method: credit card, Apple Pay, or Google Pay

You do NOT need to upload medical records, ESA letters, training certificates, or proof of disability. The ADA explicitly does not require these for public-access purposes, and we don't collect them.
