Psychiatric Service Dog Registration

He/she is vaccinated for rabies.

To my knowledge, he/she does not have fleas or ticks or a disease that would endanger people or other animals.

He/she has been trained to do work or perform tasks to assist me with my disability.

He/she has been trained to behave in a public setting.

I understand that a properly trained dog remains under the control of its handler. I understand that a properly trained dog does not act aggressively by biting, barking, jumping, lunging, or injuring people or other animals. It also does not urinate or defecate on the aircraft or in the gate area.

I understand that if he/she shows that it has not been properly trained to behave in public, then the airline may treat him/her as a pet by charging a pet fee and requiring him/her to be transported in an FAA-approved pet carrier.

To the best of my knowledge, he/she has not behaved aggressively or caused serious injury to another person/dog.

I understand that he/she must be harnessed, leashed, or tethered at all times in the airport and on the aircraft.

I understand that if he/she causes damage, then the airline may charge me for the cost to repair it, as long as the airline would also charge passengers without disabilities to repair the similar kinds of damage.

I am signing an official document of the U.S. Department of Transportation. My answers are true to the best of my knowledge. I understand that if I knowingly make false statements on this document, I can be subject to fines and other penalties.

Service Dog Agreement: I acknowledge that my completion of this form is to voluntarily register my Service Dog into US Service Animal Registrar’s registry. Doing so will provide me with identification materials to help others identify my Service Dog. Furthermore, I certify that my Service Dog has been trained to assist me with my disability. I am not registering for fraudulent misrepresentation of a Service Animal, and understand that my dog must meet the legal requirements in order for this registration and identification materials to be valid and to receive the privileges, including access into housing and public places with a Service Animal. Additionally, I understand that both federal and local laws regarding Service Dogs are subject to change in the future, and I will continue to abide by all current regulations. Should any access or privilege issues arise, I hold USSAR, LLC and its agents harmless, and understand USSAR, LLC can provide no guarantees that landlords or any other third parties will be compliant and non discriminatory with access and privileges protected by laws regarding services animals. Once submitted, customized items cannot be cancelled and are not subject to a refund. Orders lost, stolen or returned due to invalid addresses or relocation are non-refundable and are subject to a reshipment fee starting at $9.99. Furthermore, I certify that I have or will get a signed letter and/or script from a licensed Mental Health Professional or my Physician to have an Emotional Support Animal. I am not registering for fraudulent misrepresentation of an Emotional Support Animal, and understand that my dog must meet the legal requirements in order for this registration and identification materials to be valid and to receive the privileges, including access into housing and public places with an Emotional Support Animal.
